Our practice recognizes the value of integrating mental health care and primary care services to best meet the needs of our patients. We have a Psychiatric Nurse Practitioner, Candyce Miller, who specializes in medication management of mental health concerns such as depression, anxiety, bipolar disorder, PTSD, OCD, ADHD and many others for ages 18 and up. If you would like to make an appointment with her, please call the office at 615-859-1440 to schedule and also complete the Psychiatric History Form below.

We accept most public and private insurance plans. (We do not accept TennCare or Cigna Connect.) We also do our best to work with cash-paying patients. Call for more information.
