Meg Parpart, PA-C
Meg has been a Physician Assistant at Long Hollow Family Practice since 2023 when she graduated from Trevecca Nazarene University. She is originally from Northwest Indiana and got her undergraduate degree from Valparaiso University. During PA school, she spent time working at a hospital in Ghana and has a passion for medical missions. She hopes to one day do full-time missionary work in Africa. She enjoys camping, crocheting, fishing, and petting goats.

Molly Kroeger, PA-C
Molly is a Physician Assistant that graduated from the University of Tennessee Health Science Center Physician Assistant Program in December of 2023 and started with Long Hollow Family Practice shortly after. During her time at UT, she completed a certification in social determinants of health and has a special interest in women’s health. She has lived in Nashville all of her life and attending Lipscomb University for her undergraduate education. In her free time, she enjoys hiking, cooking, and is a big Titans and Tennessee Volunteers fan.
Candyce Miller, FNP-BC, PMHNP-BC
Candyce has been a Nurse Practitioner at Long Hollow Family Practice since its creation in 2002, and has been working with Dr. John Baites, the practice's founder and leader, since 1997. Candyce received a Bachelor's Degree, a Master of Science in Nursing, and a Post-Master's Certificate all from Vanderbilt University. She is dual board-certified as both a Family Nurse Practitioner and a Psychiatric Mental Health Nurse Practitioner and focuses her patient care at Long Hollow Family Practice on the psychiatric needs of our patients. She enjoys spending her free time cooking, exercising, reading, and spending time with her 2 dogs, husband, and 3 children.

Dr. Ted J. Miller, MD
Dr. Miller has been a physician at Long Hollow Family Practice since 1986. He graduated from Vanderbilt Medical School in 1982. He completed his Family Medicine Residency at Wright State University as well as one additional year of Internal Medicine Residency at Vanderbilt. He became affiliated with MDVIP in 2017. He enjoys gardening, wood working, volunteering at Cottage Cove, and spending time with his wife, 3 children, and 5 grandchildren.
Meredith Dunham, FNP-BC, MSN, RN
Meredith has been a Family Nurse Practitioner at Long Hollow Family Practice since 2003. She immediately started her career here after graduating from Vanderbilt University with a Masters in Nursing. Prior to this, she received her Bachelor of Science in Biology from Belmont University. As a native of Nashville, she enjoys church, spending time with family and friends, nature, and coffee.
Jeannie Swafford, PA-C
Jeannie began working at Long Hollow Family Practice in 2009. She received her Masters Degree from Travecca University and her Bachelor Degree from Union University. She enjoyes spending time with her husband and three children. She also enjoys coffee, couponing, and going to church.

Melissa Morazzi Carter, PA-C
Melissa has been a part of Long Hollow Family Practice since 2005. She graduated from Trevecca Nazarene University in 1994. After discovering her passion for diabetes, she went to work with a diabetologist in 2001, where she was able to perfect caring for the many facets of the diabetic patient. She also sees patients of all ages with a variety of medical problems. She enjoys teaching and has been a preceptor with Trevecca since 1998. She is married with 3 children. She loves travel, gardening, camping, photography, and studying church history.
